Take The Bridge (TTB) is conducting a new project in which we would like to include the photo/video image and/or voices of staff members and/or the general public.

I hereby authorize TTB and its agents to utilize my photo/video image and/or voice recordings in its marketing and communication materials.

I understand that photograph(s) and/or audio/visual recordings of me may be used in a presentation, publication, print advertisement, direct mail piece, electronic media (i.e. video, CDROM, World Wide Web, etc.) or other form of Communication.

In giving my consent, I hereby release and hold harmless TTB and their agents, employees, officials, representative and contractors from any and all responsibility or liability for damage of any kind suffered in any manner whatsoever.

I hereby relinquish any and all personal and proprietary rights I may have in connection with such use. I understand that I will receive no compensation should any photos/videos of me by used.

This information is being collected under the Authority of Section 33 (c) of the Freedom of Information and Protection of Privacy (FOIP) Act and may be used for the administration of TTB programs including marketing and communications events and materials. Your personal information is protected by the privacy provisions of FOIP.
